What to Bring to Your First Appointment
If possible, all new patients should bring the following to their first appointment:
- All medications you are currently taking;
- Recent MRI reports;
- Drivers License;
- Insurance Cards
New Patient Forms
Our new patients should be prepared to fill out a few forms and input medical history information into our tablet-based EMR system.
